ACCIDENT INVESTIGATION

TRAINING OBJECTIVES

At the completion of this training session each successful participant will be able to:  

- investigate a specific accident and identify the variety of initial
   causes that contributed to the undesired results. 
- use a series of questions to discover the root cause(s) of an accident. 
- follow a logical procedure to conduct effective interviews with witnesses 
- develop recommendations which can help prevent similar accidents.

        

TRAINING TOPICS

Understand your duties and liabilities related to accident investigation reports for both the Ministry of Labour and WSIB.

Steps in an effective accident/incident investigation.

 Site Investigation

-Interviewing witnesses 
-Report writing including recommendations 
-Follow up review by the committee 

 

TRAINING METHODS

A variety of presentation techniques will be utilized to promote the learning process. Training aids will include the use of overheads, videos and flip charts. Maximum involvement of the participants will be promoted by group discussions, case studies, and role playing.

 

A comprehensive accident investigation form developed by Laing Training Systems,will be used in the training session. This will allow the participants to gain practical experience conducting an accident investigation in a controlled environment. 

 

LENGTH OF WORKSHOP

The full  day version provides opportunities for in-depth discussion and exercises which promote understanding of the significance of the various steps in the accident investigation process.

 

The half day version is designed to be a refresher workshop which presents an overview of the key concepts but reduces the opportunity to internalize the steps in the process.
